I have read hundreds of posts all over google, videos, etc. But nothing is clear to me …
Is it possible in mame to use a default overlay for vertical & horizontal? or should we go game by game configuring?

MAME itself can do it (that is, you can configure the mame-libretro core to do it through the core’s own OSD menu), but RetroArch has no mechanism for discerning which games are horizontal and which are vertical.
So, you can use MAME’s built-in artwork feature to do it, but you’ll also need to use the “alternate render” core option or else your artwork will be chunky/low-res.
Okay, so I plug in a keyboard and go into mame’s osd … as I normally do, right?
I also try to try this, adding bios ini folder etc.
Yeah, scroll lock will get you into “game focus mode” where you can use tab to go into the regular MAME menu as normal.
